ICE - Medical form

Anyone attending cyclekarting events might like to print and complete this "In Case of Emergency" medical form (its the same form in MS Word and PDF formats).  The recommendation is that you bring it in a sealed envelope with your name on the outside and we can have a box to store them in during the event.  That way, should anyone need urgent medical attention, someone can hand the envelope to the relevant medical personnel so that they will have the key information they need.

At the end of the event, you take the envelope back.

Good move Stefan.
How many have ICE numbers in their phone saved as such? The medics go there very quickly. Difficulty is when you have a locked phone!

On a lot of phones you can have emergency contacts which can be accessed even if the phone is locked

Thanks for the pointer Bill, I've added an emergency contact on my phone.

I do recommend people fill out a paper version too because that's easier to put in a shared location that everyone knows about (rather than trying to find someone's phone), can be handed over to medical staff, and is unlikely to run out of battery ;)

I raced motocross in my youth and all riders had to wear a tag ( dog style ) with name on and blood type. Just a suggestion !
